I knew I needed a letter from the immigration
I have had problems in the past getting the letter of residency from immigration. On one occasion, they gave it to me just showing up with the completed form and a signed copy of my landlord's I.D. card and house registration without any problem. On a subsequent occasion, I spent weeks going back and forth with different documents without any success. I finally just sent my girl friend down and, after a small contribution to the policeman's benevolent association, he issued the letter.
Anyway, you can download the form from http://www.imm.police.go.th/. The form is called "APPLICATION FORM FOR RESIDENCE CERTIFICATE IN THAILAND." There is no "T.M" form number. The old form was about three pages and you needed to have your landlord certify your residence and attach a copy of his/her ID card and house registration along with your original passport and a signed copy of your passport. They must have changed the form and procedure though.
I think that in the future I will just get the affidavit from my embassy which is available for 1,000 Baht. The letter from immigration is supposed to be free but considering all of the problems I have had I'm not sure how "free" "free" is.
Good luck and let us know how it goes.
