Can anyone advise where I can download a copy of the Loas Visa Entry Form so I can print and complete to save time at border...

There are actually 2 forms.
The Application for a Visa
The Entrance & Exit slip/s.

The second item is a bespoke form, 2 sided with perforations (currently sponsored by Phone M).
The simple fact is that if you attempt to use one that you have 'created' yourself you will get all sorts of grief from the Immigration Officer if/when he spots it.
It is just not worth the hassle.
Turn up at Window B & take the forms, then fill them in.
If you are VERY lucky & use the route often then (on a good day) one of the Officers may give you a 'spare' set....

Thanks Dero what I thought would be a time saving exercise is not to be back to Pan A....
