Need to get pages entered into my passport, a few questions (if anyone has the answers):

Extra Pages form, will they have them or should I print one out before hand?
Has anyone had them enter pages and mail (Thai Post Office) back to Udon?? I already have a envelope w/ postage for tomorrow, just worried about the Thai mail system and how long will it take.
This will be my second set of pages entered, is there a limit?? Passport is in good shape.

Any help/info will be appreciated.

From the consulate newsletter:
Extra visa pages ($82 or Baht 2,706). Note: we can accept
applications, but the passports will have to be brought back to the
Embassy for final processing the next day. The passports will then have
to be picked up in person (or by a friend upon presentation of a letter
of authorization signed by the applicant) at the Embassy.

Got a affidavit from US embassy worked great.
What type of documentation do you have to provide the Embassy?
What is the fee?
Wouldn't the affidavit you got in December, still be valid?

The ACS outreach is a GREAT service. It has saved me a lot of hassle over the years. Do any other Countries offer a similar service for their citizens in Thailand?

Correct. Seems like 50-75 or so folks show up each time, but I've never waited more than 15 minutes or so. Good service and saves a trip to Bangkok!
