2011 is when I obtained my Thai 1 year D/L on a tourist visa admitted to stay until 12/03/2011

You are not in Thailand on a Non-O visa.
Nobody is ever staying in Thailand using just a visa.
A visa never gives permission to stay in Thailand.
The visa only allows you to get a permission to stay from an immigration officer.
None of the stamps you show are visas.
The rectangular stamps have a line Visa class with a hand written Non-O they are your permission to stay. You got them because you had a visa that was marked used (if it was a single entry)
A visa stamp if you got it at immigration in Thailand or visa sticker if from outside Thailand is much bigger and typically takes a complete page in your passport.
If you got a visa stamp in Thailand it will have immediately been marked as used as the officer will have given you a permission to stay stamp immediately after stamping the visa.
if you got a single entry visa it will have been stamped as used when you entered Thailand.

You are stubborn
To get that admitted Non O entry you need a valid visa
I got Non O multiple entry ones and none of those have been stamped as used
1 every 15 month or there about for the last 10 years till covid stopped entry
the last one does have remarks written in Thai

You are stubbornly not understanding how the system functions but why you don't isn’t easy to know.
Correct, but once given an entry stamp [admitted until/Permission to stay] your visa has no relevance whatsoever.
Congratulations and that they have never been stamped used is not at all surprising as you must never have entered on the last day of validity
Again congratulations, though I would not have liked the 40 to 50 trips outside Thailand, but if you did more power to your elbow.
I can't know what is written but others have been told that they can not continue getting visas so can only speculate that it may be something like that for themselves for your next visit.

So when you do your extension to stay how do answer Q3
Q3 Valid visa category NON-IMMIGRANT "O" or "B"
This visa can only be obtained from a Thai Embassy or consulate outside of The Kingdom of Thailand
Now when I have done mine I enter the last visa number that was given
Same as I do for 90 day reporting
Yes it has expired for entry to the country way back in March 2020 but that is the visa that is required

At our initial visit to the DLT office, Nong Han, was advised I would need photocopies of expiring D/L, yellow book, passport and visa, as well as the originals and medical certificate. For "visa", I copied the passport page stamped "Extension of stay permitted until ...." Just advising members what worked for me in May 2021. Tom

Naturally I give the information they are asking for, which is the visa used to get permission to stay that has been extended since then. In my case it was stamped used since it was a single entry Non-O
You are incorrect, you can get a Non-O visa from most immigration offices if you fulfil the requirements, correct for a Non-B
Certainly it is as you have extended the permission to stay since then.
That doesn't mean that it gave you permission to stay in Thailand

but all the multiple re entry ones no
as for incorrect it must be the Thai pamphlet that is incorrect as copied from their latest one

As I said the ME visa is extremely unlikely to have a used stamp.
Immigration is using Thai language for all decisions.
English language leaflets are purely a general guide.
Translation from Thai to English is not exact and you will find various different translations of the same original all equally correct or incorrect.
Is it surprising that a leaflet in a foreign language that is just a guide has worded that is a translation that many people (including immigration officials) agree is wrong.
Specially because there are concepts that are completely the same in one language but completely different in the other.
There are numerous translation mistakes in immigration leaflets. Congratulations on finding some.

Checking my Thailand license, I see quite clearly, in English, the words "Driving License". I believe that the equivalent document is called "drivers license" in the USA. TIT !
