COVID Vaccination Booklet

You can get your Covid vaccination booklet (yellow booklet) at the Amphur Muang Udon Thani Public Health Office, located on Athipbodi road across from the Tessaban. It only takes 15 minutes and costs 50 baht. Bring your passport plus 2 copies, your vaccination certificate and 1 copy, fill out the form at the office.
