Yes, It was a mistake,
but in the UK they keep on mentioning that the training that the training of the US pilots was not of the same level as the training of UK pilots.
If this is true (which I don't know) than I think this is wrong. this means they can try to decrease the possibility of mistakes and human errors by more and better training!!!!
I think that everything that is possible should be done in order to avoid as many mistakes as possible.

It would be very difficult to blame the pilot.
He and the other fighter pilot were both on their first missions. One would have assumed that perhaps a better approach would have been to have an experienced pilot teamed up with a virgin pilot, then perhaps the ID'ing of the friendly force orange markings on the roof would have been correctly confirmed. However the pilot did do the right thing by calling in on likelihood of friendlies being in the area, to which he had a confirmation that there were none.
What I can't quite understand was that the soldiers widow was calling for justice? I assumed that the justice would be that the thing is brought out into the open and acknowledged that some stuff up's did occur, systems are reviewed and procedures are improved to lessen the possibility of the same occuring more often than it should. And I am sure that would have been done after the event regardless of all the hush hush.
I hope it is not that she expects that someone should be taking the can for her husbands life.

Maybe a huge cash settlement would help ease some of the pain and grief.Justice ,especially during the immediate grieving period.relates to answers and someone taking responsibility!!Everyone makes mistakes,it doesn't take away the sense of loss and feelings of injustice or unfairness that the surviving family feels from their loss! 
Whats pees me off - is the denial by the governments that this tape ever existed.

It was to pay for government and military salaries to prevent near bankrupcy and was perfectly summed up by a US politician " who in their right mind would send 12 billion in cash to Iraq" ? hmmmmmmm.
