I did mine a couple of weeks ago at Telewiz in the complex (3rd floor huge shop) they did not want any id or anything - just a simple form which they fill in your number and ur name - thats about it (I am AIS (1-2-call) ) and thats where the ladies in the phone shops downstairs told me to go.

Apparently - you are supposed to register all mobile phone sim cards in thailand before the end of the year (or earlier if Wombat is correct)
This is to stop you using your phone to blow up a bomb in the South.
They say that if you do not register your sim by this time, you will be permanently disconnected.
Some cynical people would say there are other reasons for doing it - especially as a certain person owns one of the largest phone companies in Thailand - and this kind of information would be of great use to him Names addresses and service provider - but of course that is just what other people say ! how would I know ?

If someone can confirm the date please post here. I have a AIS card with 817 baht still on it that doesn't expire until Dec 22. I arrive at Don Muang on the 18th so I was going to get a 50 baht recharge in the airport to keep in going and register the card the next day. I sent an email to the AIS (1-2-Call) service center and asked them aobut my SIM card possibly expiring in November. The Nov deadline is just for the 3 southernmost providences otherwise the deadline is 31 Dec. I guess I'm safe and have tome to get my card registered.

Thank you for your e-mail. We are glad to answer your question as follows:
This policy is issued for a security reason in Thailand and it is applied to all mobile phone operators in Thailand. However, if you would like to proceed this, you need to bring along an ID card together with SIM card at AIS branches & Outlets nationwide and Telewiz. As this policy relates to a security reason so Thai government requested a cooperation from all parties including people and distributor. This must be proceeded according to government policy and from a rule of Corporation Public Company Limited or (TOT) by December 31, 2005.
In addition, if you want to use your SIM card in the specific areas like Pattanee, Yala and Naratiwas, you have to identify yourself by November 15, 2005. If you do not do within the mentionable period, we will disconnect the service.
Additional Information : For more information of product and serive of One-2-Call you can visit website www.one-2-call.com that you can also apply for Freedom club and you will be able to check balance and validity or join in events held by AIS.
